About the Role

Forest Park Hospice is looking for a compassionate and clinically strong On-Call Registered Nurse to join our dedicated hospice team. This position requires the nurse to be available for a 7 days On, 7 days Off rotation, providing after-hours support to patients and families in their homes and care facilities. This role is ideal for a nurse who values autonomy, critical thinking, and meaningful end-of-life care. The nurse will collaborate closely with interdisciplinary teams to assess, plan, implement, and evaluate patient care plans, ensuring adherence to clinical protocols and standards. This role demands strong clinical judgment, effective communication, and the ability to work independently under varying conditions. Ultimately, the On-Call Registered Nurse contributes to improving patient outcomes and enhancing the overall healthcare experience within the facility or community served.

Minimum Qualifications

  • Current and valid Registered Nurse (RN) license in Oregon.
  • Completion of an accredited nursing program (Associate's or Bachelor's degree in Nursing).
  • Basic Life Support (BLS) certification.
  • Demonstrated clinical experience in a healthcare setting, preferably in acute care or similar environments.
  • Ability to work a 7 days on, 7 days off on-call schedule with reliable availability.

Preferred Qualifications

  • Experience working in hospice/end-of-life care.
  • Experience working in on-call or shift-based nursing roles.
  • Familiarity with electronic health record (EHR) systems.
  • Strong background in patient education and care coordination.

Responsibilities

  • Respond promptly to after-hours calls and triage all incoming calls
  • Provide in-person visits for symptom management and urgent concerns in accordance with company policy
  • Support and educate families during times of crisis with calm, compassionate guidance
  • Collaborate with the interdisciplinary team to ensure continuity of care
  • Perform comprehensive patient assessments, monitor vital signs, and document clinical information accurately.
  • Maintain compliance with healthcare regulations, infection control standards, and facility policies.
  • Participate in quality improvement initiatives and contribute to maintaining a safe care environment.

Skills

The required clinical skills enable the nurse to perform thorough patient assessments, administer treatments safely, and respond effectively to emergencies during on-call periods. Communication skills are essential for collaborating with healthcare teams and educating patients and families, ensuring clear understanding and coordinated care. Time management and organizational skills help the nurse prioritize tasks and maintain accurate documentation under variable and sometimes high-pressure conditions. Preferred skills such as proficiency with electronic health records streamline documentation and information sharing, enhancing efficiency and accuracy. Advanced certifications and experience further empower the nurse to handle complex clinical situations confidently and contribute to continuous quality improvement.

Sat & Sun - 8:00 am to 8:00 am
