TERMS OF SERVICE – BOOKING & CANCELLATION FEE
1. Booking Requests
By submitting or confirming a service request with Velgato Health (“Velgato,” “we,” “us,” or “our”), the client (“Client,” “you,” or “your”) acknowledges and agrees to the terms outlined below.
Once a service request has been booked and accepted by Velgato Health, resources may be allocated to fulfill the request, including coordinating healthcare professionals, scheduling, administrative processing, and other related services.
2. Service Fee
Velgato Health charges a $98 service fee per booked request.
The $98 service fee applies when a Client cancels a booked request more than 15 minutes after the request was booked/confirmed, regardless of the reason for cancellation.
3. Cancellation Policy
Clients may cancel a booked request within 15 minutes of booking without incurring the $98 cancellation/service fee, subject to any other applicable terms (see service agreement contract))
If the Client cancels the request after the 15-minute cancellation period has elapsed, a $98 service fee will be charged.
The $98 fee applies regardless of the reason for cancellation, including, but not limited to:
Change in Client or patient needs
Change in schedule
Patient no longer requiring services
Services obtained from another provider
Insurance or authorization issues
Staffing or scheduling changes requested by the Client
Any other reason initiated by or attributable to the Client
4. Acknowledgment of Fee
By booking a request with Velgato Health, the Client acknowledges that the $98 service fee is associated with the administrative and coordination costs related to the booked request and agrees to pay the applicable fee when cancellation occurs after the 15-minute cancellation period.
5. Payment Authorization
Where applicable, the Client authorizes Velgato Health to charge the $98 service fee using the payment method or billing arrangement provided by the Client.
If the fee cannot be collected at the time of cancellation, the Client remains responsible for payment of the applicable amount.
6. Exceptions
Any waiver, refund, or exception to the $98 service fee must be approved by Velgato Health in writing. Nothing in this policy is intended to waive or limit any rights or remedies that cannot legally be waived or limited under applicable law.
7. Acceptance of Terms
By submitting, approving, or booking a service request with Velgato Health, the Client confirms that they have reviewed, understood, and agreed to this Booking & Cancellation Fee Policy.
