Job Description

We are seeking a driven and compassionate LVN to join our team. You will be a great fit if you enjoy building a relationship with your clients and are committed to providing exceptional service. An LVN will provide one-on-one care and perform following duties:

• Provides follow -up to clients after being discharged from a hospital or rehabilitation, skilled nursing or long-term care facility or under hospice care.

• Provides nursing visits in accordance with the written plan of treatment and all subsequent orders that authorize changes.

• Gathers and maintains documentation, as required by agency, for accurate and complete medical records.

• Conducts and pursues the business of the agency in accordance with sound nursing and medical practices.

• Performs nursing services only under the guidance and supervision of a registered nurse.

Requirements for our Qualified Candidates:

• A Graduate of accredited practical/vocational nursing program.

• Licensed Practical/Vocational Nurse in Texas.

• Maintain valid CPR certification while employed in a clinical role.

• One (1) year clinical experience and one (1) year home health experience.

• Texas Driver License in good standing, auto insurance and reliable personal transportation.
