RN

We are seeking a compassionate and skilled Registered Nurse (RN) to join our Home Health team for SOC opening. The ideal candidate will provide high-quality nursing care to patients in their homes, ensuring their comfort and well-being. This role requires a strong background in various nursing specialties, including critical care, urgent care, and emergency medicine. The RN will work collaboratively with a multidisciplinary team to develop and implement individualized care plans tailored to each patient's needs.

Duties

• Conduct comprehensive assessments of patients' health status and needs in their home environment.

• Develop, implement, and evaluate individualized nursing care plans based on patient assessments.

• Administer medications and treatments as prescribed by physicians.

• Provide education and support to patients and their families regarding health management and disease prevention.

• Perform specialized nursing procedures such as ostomy care, wound care, and administration of dermal fillers when necessary.

• Monitor patients for changes in condition and respond appropriately to emergencies.

• Collaborate with healthcare professionals including physicians, physical therapists, and social workers to coordinate patient care.

• Maintain accurate documentation of patient progress and interventions in compliance with regulatory standards.

Experience

• Current RN license in the state of practice is required.

• Previous experience in home health care or a related field is preferred.

• Proficiency in critical care, urgent care, emergency medicine, or laboratory settings is highly desirable.

• Knowledge of physiology and sonography principles is a plus.

• Strong interpersonal skills with the ability to communicate effectively with patients, families, and healthcare teams.

• Experience in nursing home settings is an advantage but not mandatory.
