Job Summary
We are seeking a compassionate and dedicated CHHA to join our healthcare team FOR HOME HEALTH VISITS. The ideal candidate will provide essential support to patients in various settings, including medical offices, outpatient facilities, and home care environments. This role involves assisting with daily living activities, ensuring patient comfort, and maintaining compliance with HIPAA regulations. A strong background in geriatrics is preferred.

Responsibilities

• Assist patients with daily living activities such as bathing, dressing, and grooming.

• Monitor and record vital signs and report any changes to the nursing staff.

• Provide emotional support and companionship to patients.

• Ensure patient safety by lifting and transferring individuals as needed.

• Maintain a clean and organized environment for patient care.

• Adhere to HIPAA regulations to protect patient confidentiality.

• Utilize electronic health record systems such as Epic for documentation purposes.

• Collaborate with healthcare professionals to deliver high-quality patient service.

Requirements

• Certification as a CHHA

• Ability to lift and move patients safely and comfortably.

• Strong understanding of patient care principles, particularly in geriatrics settings.

• Familiarity with HIPAA regulations and commitment to maintaining patient confidentiality.

• Experience in home care or outpatient environments is a plus.

• Excellent communication skills and the ability to work effectively within a team.

• Compassionate demeanor with a genuine desire to help others.
