Job Summary

Grove Park Healthcare Center, is seeking a dedicated and compassionate LPN Licensed Practical Nurse to join our team at our state-of-the-art facility in East Orange, New Jersey. As a member of our interdisciplinary care team, you will play a vital role in providing exceptional patient care and support to our residents.

Job Responsibilities

• Provide direct patient care, including administering medications, wound care, and vital sign monitoring
• Monitor and report patient conditions to the supervising physician and other members of the healthcare team
• Develop and implement individualized care plans in conjunction with the interdisciplinary care team
• Participate in care planning and discharge planning for residents
• Maintain accurate and detailed records of patient care and treatment
• Collaborate with other healthcare professionals to ensure quality and coordinated care
• Demonstrate a commitment to delivering compassionate and empathetic care to our residents

Requirements

• Current LPN licensure in the State of New Jersey
• Strong communication and interpersonal skills
• Ability to work effectively in a fast-paced environment
