Licensed Vocational Nurse - LVN - In Home Shift Care

Job Overview

We are seeking a compassionate and reliable Licensed Vocational Nurse (LVN) to join our home health team. The LVN will provide skilled nursing care to patients in their homes, supporting their health, safety, and overall quality of life. This role requires strong clinical skills, excellent communication, and a patient-centered approach.

Responsibilities

         Provide skilled nursing care in accordance with the physician’s plan of care

• Perform patient assessments and monitor vital signs

• Administer medications and injections as ordered

• Perform wound care, catheter care, trach care, and other skilled nursing procedures

• Educate patients and family members on disease processes, medications, and home care

• Accurately document visits, assessments, and patient progress

• Communicate changes in patient condition to the supervising RN and physician

• Follow all agency policies, state regulations, and infection control standards

• Maintain patient confidentiality and professional boundaries

Required Skills & Qualifications

• Active LVN license in good standing

• Minimum 1 year of nursing experience (home health preferred)

• Knowledge of home health documentation and patient care standards

• Strong assessment and clinical skills

• Excellent communication and interpersonal skills

• Ability to work independently and manage time effectively

• Reliable transportation and valid driver’s license

• Current CPR certification

• Basic computer skills

Preferred Qualifications

• Home health or hospice experience

• Experience with wound care, G-tubes & trachs

Patient is bedbound and wheelchair dependent. Requires skilled medication administration, assistance with feeding, and comprehensive support with activities of daily living (ADL's)

Shift details:

Skilled Nurse frequency: Sunday- Thursday 10pm-6am

 Must be able to start immediately.
