Licensed Vocational Nurse - LVN - In Home Shift Care

We are seeking a dedicated and compassionate Licensed Vocational Nurse (LVN) to join our healthcare team for home health SHIFT CARE FOR A 9-YEAR-OLD KID. The ideal candidate will provide high-quality patient care in various settings, including hospitals, nursing homes, and assisted living facilities. You will be responsible for supporting patients with their daily needs, administering medications, and collaborating with healthcare professionals to ensure optimal patient outcomes. Your expertise in critical care, geriatrics, and various medical practices will be essential in delivering exceptional care to our patients.

Responsibilities

  • Provide direct patient care, including administering medications and treatments as prescribed.

  • Monitor and record vital signs and patient health status.

  • Assist with physical examinations and diagnostic evaluations.

  • Collaborate with physicians and other healthcare team members to develop and implement patient care plans.

  • Conduct patient assessments and document medical histories accurately.

  • Educate patients and families on health management, medication administration, and care plans.

  • Maintain accurate medical records in compliance with HIPAA regulations.

  • Perform procedures such as IV insertion, catheterization, phlebotomy, and specimen collection.

  • Support patients with activities of daily living (ADLs) in various settings including long-term care and memory care facilities.

  • Participate in discharge planning and case management to ensure continuity of care.

Skills
Candidates should possess a range of skills that enhance their ability to provide excellent patient care. Ideal qualifications include:

  • Proficiency in using EMR/EHR systems such as Epic, Cerner, Athenahealth, or eClinicalWorks.

  • Experience in critical care settings such as ICU or emergency medicine.

  • Knowledge of ICD-9/ICD-10 coding for accurate medical documentation.

  • Familiarity with geriatrics, dementia care, Alzheimer’s care, hospice care, and senior care practices.

  • Competence in acute pain management techniques and medication administration.

  • Strong understanding of anatomy, physiology, medical terminology, and infection control protocols.

  • Ability to work effectively with individuals with developmental disabilities or behavioral health needs.

  • Experience in pediatrics or working with children is a plus.

9-yo medically fragile pediatric patient: bed-bound with no head control, scoliosis w/ spinal rod, seizure disorder, diabetes, and incontinence. Requires G-tube feeds, multiple meds, CPAP, suctioning, and very gentle handling. Has ongoing PT and medical appointments.

40 hours A WEEK, YOU WILL NEED TO START ASAP
