Role Description

This is a contract on-site role for a Registered Nurse (RN) with expertise in home care, case management, or a supervisory background, located in Maplewood, NJ. The RN will oversee home care services, manage patient caseloads, coordinate care plans, and ensure the highest quality of patient care. Responsibilities include conducting timely assessments, implementing care plans, providing wound care, and maintaining communication between patients, caregivers, and healthcare providers. Collaboration with team members and upholding compliance with healthcare regulations are essential aspects of this role.

Qualifications

• Nursing skills, including patient assessments and care planning
• Expertise in Medicine and Patient Care, with a focus on personalized treatment
• Experience in Home Care and Wound Care to support patient recovery processes
• Strong communication, organizational, and problem-solving skills
• Ability to work collaboratively within a team and build strong relationships with patients and their families
• Current RN license in the state of New Jersey
• Previous experience in case management or supervisory roles preferred
• Proficiency in maintaining detailed and accurate documentation
• Familiarity with healthcare compliance standards and best practices
