POSITION SUMMARY
Responsible for a wide range of community-based outreach, counseling, education, and organizing activities relating to HIV/AIDS, with the primary mission to ensure early intervention and enrollment into healthcare programs and ADAP and other types of health coverage. They will participate in training and support activities, and utilize data forms to document and report activities performed. They will provide services outlined in CHN’s ADAP Education and Enrollment Initiative in Upper Manhattan and Bronx.
DUTIES AND RESPONSIBILITIES
- Works with Program Manager to assess community needs around engagement and enrollment into ADAP and other health coverage programs, and to develop and design campaigns and materials.
- Conducts street outreach and door-to-door outreach in targeted areas identified by Project Manager to meet grant targets.
- Provides on-site workshops and outreach activities.
- Participates in community health and street fairs as an agency representative.
- Engage community residents in preliminary screenings and assessments for enrollment in ADAP and other types of health coverage while in the field.
- Provides referrals, follow-up and/or escorts clients to necessary health and support services in the community, including making appointments for clients at CHN clinic sites, and making appointments for health coverage assessments and enrollment.
- Facilitates related services for health center patients and CHN staff as appropriate, with respect to their confidentiality and privacy.
- Identifies community resources and makes contact with such agencies, assisting with referral relationships and agreements as necessary.
- Conducts other agency outreach including mass mailing, telephone contacts, etc.
- Attend trainings, workshops, in-services, support activities and other meetings as designated.
- Utilize reporting forms accurately to document and report activities performed and enter this data into AIDS Institute Reporting System (AIRS).
- Performs other related duties, as assigned.
EDUCATION & EXPERIENCE
- High School diploma or G.E.D. preferred.
