Licensed Vocational Nurse

Responsibilities:

• Complete skilled nursing visits as ordered by the physician

• Services may include assessments, wound care, medication management, Foley care, injections, and diabetic education

• Document visits using our EMR (WellSky) – training provided

• Report any changes in patient condition to the clinical manager

• No therapy coordination required

Qualifications:

• Active  LVN license in the state of Texas

• Current CPR/BLS certification

• Reliable transportation and valid auto insurance

• Must pass a background and clearance check

• Home health experience is preferred, but not required
